29 CFR 1910.132(f)(1)(iii): Employee(s) required to use PPE by this section were not trained to know how to don, doff, adjust, and wear PPE: On or about 03/05/2020, Toms Tree Service LLC, located at 1313 S. Main Street, Albia, Iowa, the employer did not ensure that the Bucket Operator at 6957 Weldon Spring, MO, had donned the personal fall arrest harness properly. The leg straps were not engaged while in an elevated aerial lift bucket. The importance of donning the harness properly with the leg straps had been ignored or forgotten since in an aerial bucket. An incident occurred with the victim 20 25 feet above ground level and was tossed out of the aerial lifts bucket.
