29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: a) On or about January 23, 2020, at 2030 Burning Tree Lane, Youngstown, Ohio 44505, the employer did not ensure that employees working from the ground, were using head protection (hardhats) while conducting tree removal operations of an approximately 130 foot tall red oak tree. The tree removal was being conducted overhead thus exposing the employees to a struck by hazard.
