29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: a) Line 1 North Trench Conveyor in the Front End - On or about January 7, 2020, Group LOTO procedures were not utilized for the Line 1 North Trench Conveyor when an employee needed to access the Line 1 Trench to replace a belt on the Line 1 North Trench Conveyor that is located approximately 12 inches directly next to the Line 1 South Trench Conveyor. b) Line 1 South Trench Conveyor in the Front End - On or about January 7, 2020, Group LOTO procedures were not utilized for the Line 1 South Trench Conveyor when an employee needed to access the Line 1 Trench to replace a belt on the Line 1 North Trench Conveyor that is located approximately 12 inches directly next to the Line 1 South Trench Conveyor.
