29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purpose for which they were designed: (a)Grangaard Construction, Inc. at Highway 212 Bridge, Watertown, SD 57201: On or about and at times prior to October 19, 2019, the exposing employer did not ensure that two A-frame ladder used to gain access into the cofferdams were being used in accordance with manufacturers' recommendations. The A-frame ladders were leaned against sheet piling for access/egress into the cofferdam, and not opened with the spreaders locked. This condition exposed employees to a 20-foot fall hazard into the cofferdam.
