29 CFR 1910.157(c)(4): Portable fire extinguishers were not kept in their designated places at all times except during use: On or about December 12, 2019, employees were exposed to burns and smoke inhalation because the fire extinguisher that was to be mounted and stored on the outside wall of the electrical room was kept inside the electrical room.
