29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points and rotating parts: a. On or about 12/12/2019, at the fabrication area near the small MEP bending machine, employees were exposed to the rotating guide pulleys, in that, a guard was not provided over the rotating parts. b. On or about 12/12/2019, at the fabrication area under RMS 14 bending machine, employees were exposed to rotating parts under the machine at the operator's side, in that, the guard enclosing the front of the machine was removed and not replaced. c. On or about 12/12/2019, at the fabrication area shear line 1, employees were exposed to a struck by hazard, in that, at the side of the shear line table two guards were missing exposing employees to the rotating parts at the side of the table.
