29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee used the walking-working surface: a. Site- Employees were not protected from a slip and trip hazard where a damaged fixed stair, readily available for use, and is part of the emergency exit discharge to the roof at the second level of paper machine 9, was not repaired. b. Site- Employees were not protected from a slip and trip hazard where a fixed stair, between the lower and upper catwalk along the front side of paper machine 9 at the Yankee dryer, was removed.
