29 CFR 1910.269(l)(8)(v)(D): The employer did not ensure that each employee exposed to hazards from electric arcs wore protective equipment consisting of at a minimum a face shield with an arc rating greater than or equal to the heat energy estimated under paragraph (l)(8)(ii) of this section whenever that estimate exceeded 2.0 Cal/cm2 but less than 9.0 Cal/cm2: (a) A.H. McCoy Federal Building Electrical Vault - On or about November 13, 2109 employee assisting in the performance of a phase test was exposed to arc flash burn hazard. Employee was not wearing an arc rated face shield.
