Titusville Enterprises dba Titusville Fabricators, 191 Howard Street, Franklin, PA 16323, maintenance shop area / tool room, at the work bench, on or about December 9, 2019 and times prior thereto: A flexible cord was wired from a snap switch box to a cord cap to power an overhead lighting fixture above the work bench. The flexible cord had damage to the outer jacket. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
