29 CFR 1910.119(f)(1)(iv): Safety systems and their functions. The employer did not implement operating procedures for overriding safety systems and their functions. On or around November 3, 2019, in the Acid Scrubber Area of the Glycol#2 unit at Dow Chemical Plaquemine Plant, the employers operating procedures for Changing Mod5 Program Variables were not implemented when access to the Hardware Abort Key was not restricted and the Emergency Block Valve EBV #4502 was manually opened by the Hardware Abort Key without completing the required hazards review.
