Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1926.753(d)(1): Routes for suspended loads shall be pre-planned to ensure that no employee is required to work directly below a suspended load except for employees engaged in the initial connection of the steel; or employees necessary for the hooking or unhooking of the load. On September or about 10, 2019 located near Orion station area there was a section of track with cantilevered attached catwalk identified as MCT F9065 ST8, which had the section of track resting on cribbing located on the ground with the cantilevered attached catwalk load rigged by a rough terrain forklifts elevated forks and sling attached to the catwalks guardrails while employees were underneath the cantilevered catwalk torqueing bolts when the sling was released without maintaining structural stability of the load during the process by an adequate stabilization system to prevent falling or shifting of the load causing the track section with cantilevered catwalk to overturn without pre-planning to ensure employees were not required to work directly below the load exposing employees working under the load to be exposed to potential crushing hazards by the overturning structural steel section of track with cantilevered attached catwalk.
