At the facility located at 12567 FM 3430 Vernon, Texas: the employer did not ensure periodic inspections were conducted to ensure the energy control procedures were followed. The employer did not list equipment, date of inspections and employee's performing the work. Employees were exposed to electrical shock and arc flash hazards.
