29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged. The employer is failing to prevent employee exposure to contact with electricity. The condition was most recently observed on or about August 13, 2019, at 225 Ladue Lake, Creve Coeur, Missouri. The employer did not ensure that employee(s) who were required to move an electrical wire used to power a GFCI-protected outlet to the rear wall as part of a bathroom remodel, had de-energized and attached tags to the electrical panel prior to beginning work. An employee suffered fatal injuries as a result of this condition.
