29 CFR 1910.179(j)(2)(iv): Monthly inspections of hoist chains, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and an identifier of the chain which was inspected, were not performed: On or about August 8, 2019, the employer failed to ensure Spanmaster crane, with a 2 ton Yale hoist block, model number KEL2-20RT15/5S2, serial number K1055010 and a Spanmaster crane, with a 3 ton Columbus McKinnon hoist block, model number RRT2, serial number L-052CML were inspected on a monthly basis to ensure the chains did not have safety issues.
