29 CFR 1910.146(g)(2): Training was not be provided to each affected employee: a) Jobsite Located at West Rively Avenue and South Sycamore Avenue, Alden, PA: The employer did not ensure that all affected employees in the workplace were provided training on the hazards of permit required confined spaces. The employees whose job tasks required them to access and work around sewer manholes were not adequately trained on the hazards involved, including but not limited to hazardous atmospheres, engulfment, falls and restricted means of egress. Documented to occur on or about 11 JUL 2019.
