29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Employees performing testing or positioning of machines, equipment did not perform the following: (ii) remove employees from the machine or equipment area in accordance with (e)(2) (v) deenergize all systems and reapply energy control measures in accordance with paragraph (d) to continue with servicing and/or maintenance On or about April 16, 2019, employees performing testing on the Worthington welding positioning machine #33112238 did not following step 2 and 5 of the sequence while conducting testing of the equipment.
