29 CFR 1910.147(d)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: In the production area, the employer did not develop written lockout/tagout procedures describing how to lockout and control hazardous energy sources for affected and authorized employees for machines, such as but not limited to, the Piranha Iron Worker and the Bystronics Laser machine, which exposed employees to the hazard of being caught in or struck by the equipment.
