29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them: (a) Department 126, Poletype Division, Regulator Tank Shop, Leak Test and Repair - On or about March 15, 2019, the employer did not ensure that the lockout procedures clearly and specifically explain the placement of the safety chains for the leak test device to isolate the gravitational energy source, exposing employees to struck by hazards.
