29 CFR 1926.95(c): Personal protective equipment was not of safe design and construction for the work to be performed: a. On or about March 4, 2019, at the west gun barrel tank, the employer failed to provide and ensure the use of flame resistant/retardant fall protection harness and lanyards by employees who worked at the west gun barrel tank top where a propane torch was used to heat a pipe opening of the tank top. The employees wore non-flame resistant personal fall protection devices and were exposed to a fire at the west gun barrel.
