29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees selected by, and under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration: a) 5439 Spruce Street, Philadelphia, PA - The employer permitted untrained employees to erect and work from a pump jack scaffold ill equipped of mud sills under each of the uprights and for not having an adequate fall protection system in place, thus exposing the employees to fall and scaffold collapse hazards. This condition was documented to occur on or about January 9, 2019.
