29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Stamping Department- On or about March 14, 2019, the established procedures for the application of energy control did not specify the procedural steps for dissipating the hazardous kinetic energy from the flywheel, blocking and securing the potential gravitational energy, and means of securing and dissipating stored pneumatic energy on the mechanical presses, such as Press #13, Press #14, Press #24 and Press #3673, and the Minster Press, at the facility. b) Bending Department- On or about April 23, 2019, the established procedures for the application of energy control did not specify the procedural steps for dissipating the hazardous kinetic energy from the flywheel, blocking and securing the potential gravitational energy, and means of securing and dissipating stored pneumatic energy on the press brakes, such as Press Brake #17, at the facility. c) Shear Department- On or about April 16, 2019, the established procedures for the application of energy control did not specify the procedural steps for securing the potential gravitational energy, and means of securing and dissipating stored pneumatic energy on the shears, such as Shear #1, at the facility. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
