29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface: On or about November 2, 2018 and at times prior thereto, at the jobsite, the project superintendent employees utilized the tubular welded scaffold to gain access to the roof where the roof upper landing was approximately 36 inches or more vertically from the scaffold working platform and approximately 24 inches horizontally away from the roof edge exposing employees to an unprotected fall hazard of up to 16 feet to the concrete below.
