29 CFR 1926.1060(a)(1)(v): The employer shall ensure that each employee has been trained by a competent person in the Standards covered by this Subpart: On or about October 18, 2018 and prior to, employees who were not trained on the applicable Subpart X standards, utilized the onsite elevated water tower storage tank systems caged fixed ladder with an individual ladder section total climb length of 69 feet without a ladder safety device, self-retracting lifeline, or the fixed ladders cage provided with offset ladder sections and landing platforms at maximum intervals of 50 feet exposing employees to fall potentials in excess of 69 feet.
