29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On 7/17/18, at the Acworth facility, not all authorized employees performing service and maintenance to equipment affixed a lock to each energy isolating device. i.e. at the Audit booth only 1 lock was hung for 3 employees performing service to the equipment.
