29 CFR 1926.451(g)(3): In addition to meeting the requirements of 1926.502(d), personal fall arrest systems used on scaffolds shall be attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member. The employer is failing to protect employees from a fall hazard. Two World Wrecking employees were using a single-point adjustable suspension scaffold to access an empty elevator shaft and were not protected by an adequate personal fall arrest system. Employees were not attached to independent vertical lifelines.
