29 CFR 1910.146(d)(2): Identify and evaluate the hazards of permit spaces before employees enter them; a. On or about May 25, 2018, an employee cleaning the inside of a semi-truck tanker was pronounced dead following a confined space rescue. The employee was found to have suffered chemical asphyxia by vitiated atmosphere, hydrogen sulfide and hydrogen cyanide gases. Where the hazardous atmosphere had not been eliminated prior to entry, a permit required confined space permit had not been completed and the employer had not identified the chemicals inside the tanker trailer and evaluated the hazards that the chemical mixture posed to the confined space entrant prior to confined space entry.
