29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. On or about May 4, 2018: Employees performing lock out tag out procedures on machines located in the facility were exposed to the release of hazardous energies when the specific procedures did not outline the steps for shutting down, isolating, blocking all energy sources on machines work was being performed on. a) Haas SL 20 CNC - Missing the shutting down and isolation of pneumatic or hydraulic energies. b) Fadal VMC 4020 CNC - Missing the shutting down and isolation of pneumatic or hydraulic energies. c) Fadal VMC 4020 CNC - Missing the shutting down and isolation of pneumatic or hydraulic energies.
