29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) when the operator had been involved in an accident or near-miss incident to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely: On or about April 27, 2018, at 9901 NW 106th St., Medley, FL, the employer failed to provide refresher training and re-evaluation to powered industrial truck operators after being involved in an accident or a near-miss incident with storage and pallet racks, thus exposing employees to struck-by hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
