29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Facility, 393 Industrial Park Road, Elysburg, PA - Employees performed servicing tasks, such as but not limited to preventative maintenance, on the Boiler (Serial No. 0L093176) without using lockout devices on valves (natural gas, steam, water) that isolated the Boiler from its thermal and hydraulic energy sources thereby exposing employees to struck-by and burn hazards associated with the unexpected release of energy, on or about March 19, 2018. b) Soft Side, 393 Industrial Park Road, Elysburg, PA - Employees performed servicing tasks such as but not limited to preventative maintenance, set-up, cleaning, and clearing jams on the the Shanghai Yaguang Toy Equipment Factory Carding Machine Model PKS - 900 (Picker #3) without using a lockout device on the motor controlling knife switch that isolated Picker #3 from its electrical energy source thereby exposing the employees to a caught-in hazard associated with the unexpected release of energy, on or before March 19, 2018. c) Facility, 393 Industrial Park Road, Elysburg, PA - Employees performed servicing tasks, such as but not limited to preventative maintenance, on the Popper (Serial No. 2009101) without using lockout devices on valves (air, steam) that isolated the Popper from its thermal and hydraulic energy sources thereby exposing employees to struck-by and burn hazards associated with the unexpected release of energy, on or about March 19, 2018. d) Soft Side and Hard Side, 393 Industrial Park Road, Elysburg, PA - Employees performed servicing tasks, such as but not limited to clearing jams or preventative maintenance, on the thirteen (13) Stuffers without using a lockout device on the motor controlling knife switches that isolated the Stuffers from their electrical energy sources thereby exposing employees to caught-in hazards associated with the unexpected release of energy, on or about March 19, 2018. In accordance with 29 CFR 1903.19, abatement certification and documentation is required for this violation.
