29 CFR 1910.1030(f)(1)(i): The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure and post exposure evaluation and follow-up to all employees who had an exposure incident: A) Alex Products, Inc., located at 8110 Gasser Road, Paulding, OH 45879: On or about March 6, 2018, the employer did not make available the Hepatitis B vaccination and post-exposure evaluation and follow-up to the employees who were exposed to blood-borne hazards while they assisted injured employee and cleaned the area of the wire draw machine located at the Draw Line # 2 where the employee suffered partial amputation of the right index finger.
