Text For Citation: 01 Item/Group: 003B Hazard:
29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: On or about March 8, 2018, employees were exposed to struck-by hazards when employees did not verify that lockout had been effective because: a. Employees believed that the buttons did not work because they had locked it out. b. The touch panel was blank and there was no way to use the touch screen to verify lockout. c. Employees did not know which switch was the start/stop switch that was to be pushed to verify lockout according to the machine specific procedure. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
