29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): (a) on the dry dock which was holding M/v Matanuska at Vigor Marine, LLC shipyard at 5555 N Channel Ave, Portland, Oregon: On or about 2/7/2018 and at times prior thereto, employees were exposed to struck by and rollover hazards, in that an industrial truck was being operated by a driver who was not on the list of certified PIT operators maintained by Vigor.
