29 CFR 1910.147(c)(7)(i)(A): Each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about January 24, 2018, and at times prior thereto, an employee was conducting maintenance on the Titan strapping machine without first being trained on the lockout/tagout program, exposing the employee to the hazard of being caught in the machine.
