29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about December 17, 2017 and there prior, employees replacing a gasket in a pipe with hazardous chemicals such as but not limited to Carb Off and Super Force in the Fryer Room did not affix lockout and or tagout devices on the equipment, which included a fryer and a filter.
