29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about December 16, 2017, the employer did not ensure employees implemented energy control procedures during the clearing of jams (service and maintenance activity) on the Inedible Food Plant short auger, by turning off and/or shutting down [per the 1910.147(d)(2) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to each energy isolating device [(d)(4)(i)], and verifying the equipment energy was isolated and de-energized prior to starting work [(d)(6)
