29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations. a) Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about November 17, 2017, the employer did develop or implement procedures necessary for the safe permit space entry operations when entering the pits located beneath the vertical centrifugal casting machines located in the centrifugal department. Employees entering the pits of the vertical casting machine located in the centrifugal department to install the repaired vertical caster were exposed to potential hazardous atmospheres and water engulfment hazards.
