29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Instances including, but not limited to, the following: a) Job site, 500 Scotti Drive, Bridgeville, PA: On or about October 19, 2017, during set up activities on the Optimat KAL 310 edgebander unit, the employer did not ensure that locks were applied to all energy isolating devices after they were shut off, resulting in an amputation injury occurring to an employee attempting to make an adjustment to the machine as it was operating.
