29 CFR 1910.29(b)(2): (2) Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: a. On or about August 17, 2017, located at the North Shipping Crane, Crane 257, employees accessed the crane from a walkway that had a gap of approximately 26 inches where there were no midrails installed to protect employees from falling, thereby exposing employees to a fall hazard of approximately 40 feet.
