29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a) Fresenius USA Manufacturing, Inc., on or about August 8, 2017, the employer did not ensure that the established procedures for the application of energy control for the Federal All Stainless Bottle Filler, serial number 2462, on the bottling line covered the actions listed in and was done in the sequence as required by 29 CFR 1910.147(d)(1)-(6), exposing employees to electrical and mechanical hazards.
