119(j)(4): Inspections and tests were not performed on process equipment to maintain the on-going integrity of process equipment: The employer is failing to protect employees from explosion and fire hazards by failing to perform inspections and testing to maintain process equipment in the lead styphnate manufacturing process. This was most recently documented at 25201 E. 78 Hwy, Independence, Missouri, regarding the following equipment: a) Inspection and testing to ensure mechanical integrity of equipment was not performed for the lead nitrate valve in the lead styphnate manufacturing process. b) Inspection and testing to ensure mechanical integrity of equipment was not performed for the magnesium styphnate valve in the lead styphnate manufacturing process. c) Inspection and testing to ensure mechanical integrity of equipment was not performed for the Delta V Control System Monitor and associated transmitting equipment, monitoring equipment for the lead styphnate manufacturing process.
