29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process. -Alternative Citation- 29 CFR 1910.119(e)(3)(i): The Process Hazard Analysis did not address the hazards of the process; This violation was most recently observed on or about August 3, 2017, and times prior thereto, in the VPS#2 Process Unit at Motiva Plant located in Port Arthur, Texas, when the employer did not identify, evaluate and control the hazardous condition of flammable hydrocarbon liquid and or gas potentially reverse/back flowing into the service water piping from the Atmospheric Tower's Condensers EX001 and EX005.
