29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. 918 N. Porter St. in Streator, IL 61364, warehouse, powered industrial trucks (counter-balanced propane-gas powered forklifts): on or about July 17, 2017, employees permitted to operate forklifts for regular mechanical material handling tasks were not provided with adequate operator's initial training content in truck-related and workplace-related topics that included formal instruction, practical training, and evaluation applicable to their assigned work tasks. Employees were exposed to struck-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
