29 CFR 1910.333(b)(2)(iii): A lock and a tag was not placed on each disconnecting means used to de-energize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section: On or about June 2, 2017, the employer did not ensure employees trouble shooting an electrical panel box for booster station #1 pump utilized a lock and tag on the disconnect and exposed them to electric shock hazards.
