29 CFR 1910.38(f)(3): The employer did not review the emergency action plan with each employee covered by the plan when the plan was changed. Employees engaged in operating, inspecting, testing, and maintaining activities at the covered process equipment on the west addition of the Tyson Prepared Foods facility at 9 N Washington in South Hutchinson, Kansas were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility in that the current Emergency Action and Response Plan was not updated as the facility expanded. The facility had undergone a significant capital improvement adding an addition on the west end. None of the EMERGENCY EXITS & MEETING PLACES diagrams had been updated to reflect the new construction expansion project.
