29 CFR 1910.217(d)(9)(iv): Safety blocks were not used when mechanical power press die(s) were being adjusted or repaired in the press: A) Maumee Assembly & Stamping, LLC, located at 920 Illinois Avenue, Maumee, OH 43537: On or about May 1, 2017, the employer did not ensure the use of die safety blocks while employees were setting up dies on the Verson TR-4 mechanical power press (Serial # 13558, 1200 Ton). Employees were exposed to amputation hazards while setting up dies in the press. B) Maumee Assembly & Stamping, LLC, located at 920 Illinois Avenue, Maumee, OH 43537: On or about May 13, 2017, the employer did not ensure the use of die safety blocks while employees were setting up dies on the Verson BLK1 mechanical power press (Model # S4-1500-186-108T, Serial # 24680, 1500 Ton). Employees were exposed to amputation hazards while setting up dies in the press.
