29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about February 8, 2017, located at the Blister machines in the Blister Pack Department, employees were exposed to the unexpected startup/release of stored energy and the heat of approximately 400 degree hot plate when employees performed activities of cleaning out the inside of Blister Machines, removing jammed blister preforms, and cleaning and changing out hot plates. The employer failed to implement energy control application steps as the Blister machines were not shutdown or turned off, electrical disconnects and pneumatic ball valves were not isolated with heat plate cool down times applied [per the 1910.147(d)(3) requirements], LOTO devices were not applied to isolation devices [per 1910.147(d)(4)]. The remaining applicable energy control elements involving dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing and maintenance hazards. On February 8, 2017, an employee was injured when removing a jammed blister preform while Alloyd Blister machine, model number 6S1216, serial number 93117 was still in operation.
