29 CFR 1910.333(b)(2)(iv): The employer did not properly verify if the components were positively de-energized or in an otherwise safe condition to protect the employees from an electrical shock hazard. At the work site, an employee worked on an 480 volt switch and did not verify that the switch was de-energized prior to commencement of work, exposing him to electrical shock and/or burns.
