29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a) On or about 11/8/2016, In the facility: Employees performing maintenance and die changes on equipment such as, but not limited to mechanical power presses, turret press, and a tumbler. The equipment had energy sources such as electrical, pneumatic, and potential energies. The company had not provided employees with equipment and hardware necessary to perform energy control/ lockout activities, such as, but not limited to locks, hasps, valve covers and blocking. Abatement certification is required for this item.
