29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight capacity of the truck and attachment combination at maximum elevation with load laterally centered: Thermoplastics, Hyster S50XM counterbalance powered industrial truck: on or about November 8, 2016, the truck's capacity and safe operation were affected by drilling holes in fork attachments for lifting and transporting suspended loads (molds) that were subject to swinging. The truck was not marked to identify the approximate change in weight capacity for this attachment. Employees operating equipment, and working around the moving truck with suspended loads were exposed to struck-by and/or crushed-by hazards likely to result in serious injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
