29 CFR 1910.1096(f)(1)(i): The immediate evacuation warning signal was not a midfrequency complex sound wave amplitude modulated at a subsonic frequency with a sound wave in free space having a fundamental frequency (f(1)) between 450 and 500 hertz (Hz) modulated at a subsonic rate between 4 and 5 Hz On or about 8-10-2016 employees were exposed to ionizing radiation hazards when the employer failed to provide an evacuation warning system for radioactive containment or release incidents.
